Client Update Form – For a client to update a clinician prior to or between sessions.
Document Upload – For a client to upload documents such a parenting plan, new insurance ID, a mood log, or completed Vanderbilt scales.
*Note: Information submitted will be provided to your clinician and can be discussed during your next appointment. There will not be a response provided once updates are completed. Please bring your photo id and insurance card(s) to the first appointment.
New Client Paperwork and BAPC Policies – Please complete this paperwork to request to become a new BAPC client for therapy for testing. Once completed, we will know if BAPC is a good fit for your needs. This takes approximately 20-30 minutes but can be saved and returned to if unable to complete in one sitting.
Child Developmental History – Please complete this packet for all new therapy clients under 18 years old. Must be completed by legal guardian. If separated or divorced parents, please bring a copy of the official parenting plan to the first appointment as well.
Adult History – Please fill out this packet if you are 18+ and bring to your first therapy session.
Telehealth Information and Consent – Must be completed prior to first telehealth session.
Insurance Update and Consent to Treatment – Please fill this out if seeing a new provider or your insurance has changed.
Release of Information Form – Allows records or necessary information sent to another medical office or applicable party.
